Gynaecomastia treatment in Dortmund: consultation, surgery and aftercare in one place

Your gynaecomastia treatment takes place at Westfalenklinik in Dortmund, from the first consultation to the final check-up. The specialist clinic for plastic and aesthetic surgery has been working at the same location on Rosemeyerstraße for 25 years and depends on external surgical centres or private-practice clinics neither for the operation nor for the two nights afterwards; at night you are looked after by the clinic’s own qualified staff. A personal contact accompanies you through all appointments.

Westfalenklinik is located directly on the B1/A40, very close to the Signal Iduna Park. Coming from the B1, take the Dortmund-Hombruch exit, turn right into Wittekindstraße and after 700 m right into Rosemeyerstraße; parking is on the building’s parking deck directly in front of the entrance. Without a car, take the U45 from the main station to Stadtgarten and continue with the U42 to Theodor-Fliedner-Heim.

Enlarged breasts in men: gynaecomastia

The term gynaecomastia refers to enlarged breasts in men, which in many cases already become established during puberty. Despite massive efforts by those affected (diets and exercise), the phenomenon of “man boobs” is difficult to remedy without surgery.

A distinction is made between true gynaecomastia and pseudogynaecomastia, whereby in true gynaecomastia a combination of both forms is also possible. In pseudogynaecomastia there is (only) an increase in fatty tissue. The problem of male breasts is therefore caused by “simple” weight gain. In true gynaecomastia (1) there is always an enlargement of the mammary gland, which can also be observed in slim or athletic men, for example. In addition, there may also be an increase in fatty tissue, in which case one speaks of a mixed form (2) of gynaecomastia:

  1. glandular form (mainly glandular tissue)
  2. mixed form (glandular body and fatty tissue)

In many cases, gynaecomastia develops during puberty

In most cases, gynaecomastia develops during puberty (pubertal gynaecomastia). Men affected rarely complain of physical symptoms or hypersensitivity of the breast. However, the psychological burden caused by the enlarged breasts is often considerable, which shows in the way they go about their daily lives. In many cases, gynaecomastia can only be treated surgically.

In true gynaecomastia (glandular gynaecomastia), the male mammary gland is removed (mastectomy), with the areola and the nipple of course being preserved. When the excess glandular tissue is removed, a combination with liposuction is a good option. The incision, and thus a later scar, lies in the area of the areola.

In pseudogynaecomastia, i.e. enlargement of the breast due purely to fat deposits, liposuction is usually an option. The liposuction is carried out using the classic tumescent technique. Even during the operation, the stretched skin can contract once the fat cells have been suctioned out. In the following weeks, the scar tissue beneath the tightened skin usually recedes and the chest becomes softer again. In the case of considerable excess skin, however, it is necessary to follow up with a lifting operation in a second procedure, for example after considerable weight loss with already existing excess skin that has sagged well below the inframammary fold.

The forms compared

Which form, which treatment?

Before the procedure, our doctors determine whether the breast is enlarged mainly by fatty tissue or by glandular tissue. The treatment depends on this.

Pseudogynaecomastiafatty tissue only

The breast is enlarged solely by fat deposits. Liposuction using the tumescent technique is usually an option, after which the stretched skin can contract.

Goals of gynaecomastia treatment

  • Aesthetic appearance of the chest that suits the rest of the upper body
  • Tightening of the male breast
  • Return to everyday life usually after a few days

What is the procedure for treating gynaecomastia at Westfalenklinik?

Surgical breast reduction for gynaecomastia takes about 1 to 1.5 hours and, depending on the extent, is carried out under local anaesthesia, under twilight sedation or under general anaesthesia. Before the procedure, our doctors analyse whether mainly fatty or glandular tissue is responsible for the enlarged breasts and whether, in the case of obesity, weight loss may first be necessary before the operation.

In pure pseudogynaecomastia, the tissue is loosened as part of a liposuction, a special solution (tumescent solution) is injected and the excess fatty tissue is then suctioned out with a fine cannula. During healing, the skin adapts to the flatter contour of the chest. In the more complex true gynaecomastia, excess glandular tissue is responsible for the enlarged breasts. In addition to the usual liposuction, this tissue is surgically removed in two steps.

This is how gynaecomastia treatment proceeds at Westfalenklinik, from the consultation to the end of the recovery period:

  1. Step 1

    Beforehand

Consultation in Dortmund. In the consultation, we clarify whether mainly fatty or glandular tissue is enlarging the breasts and whether weight loss is necessary beforehand. You can book your appointment online.

  2. Step 2

    Day of surgery

The procedure. It takes about 1 to 1.5 hours, depending on the extent under local anaesthesia, under twilight sedation or under general anaesthesia. Afterwards you usually stay two nights in the clinic.

  3. Step 3

    After a few days

Back to everyday life. You are back out among people. Swollen, reddened scars and bruising are possible during this time.

  4. Step 4

The first 2 to 3 weeks

Compression garment. For two to three weeks you wear a compression garment.

  5. Step 5

    The weeks that follow

The breast softens. After liposuction, the scar tissue under the skin usually regresses and the breast becomes softer again.

  6. Step 6

    Up to 6 months

Sport and sun. For up to six months you avoid sport and direct sun exposure.
