POSE® 3.0 Endo-sleeve in Dortmund: consultation, procedure and aftercare at the Westfalenklinik

The POSE® 3.0 Endo-sleeve is offered by the Westfalenklinik in Dortmund. Consultation, procedure and aftercare take place under the same roof: the specialist clinic for plastic and aesthetic surgery does not depend on external surgical centres or clinics with visiting doctors for this, and if you stay with us for the first night after the gastroscopy, you are cared for by the clinic’s own qualified staff. A personal contact person accompanies you from the first consultation to the final check-up.

The clinic is located at Rosemeyerstraße 2–4, right on the B1/A40 near the Signal Iduna Park. By car, leave the B1 at the Dortmund-Hombruch exit, turn right into Wittekindstraße and after 700 m right into Rosemeyerstraße; parking is on the building’s parking deck directly in front of the entrance. By public transport, take the U45 from the main station to Stadtgarten and continue on the U42 to Theodor-Fliedner-Heim. This also makes the follow-up appointments after the procedure easy to reach.

Personal consultation on the Endo-sleeve

In a personal consultation in Dortmund, our doctors explain the POSE® 3.0 Endo-sleeve procedure to you, its requirements and risks, and discuss with you whether it is an option for you. You can book your appointment online.

What is the POSE® 3.0 Endo-sleeve?

The new POSE® 3.0 Endo-sleeve combines the lasting effect of a surgical stomach reduction with the comfort of an endoscopy without an incision through the abdominal wall. With only 6–8 special anchors and our double-fold strategy (“Double Helix”), we reduce your stomach volume permanently, with the aim that only 25% remains, as an outpatient and with short downtime.

Many people suffer from excess weight and are severely restricted in their quality of life. This affects not only the patients’ appearance and mental well-being, but also their health. Stomach reduction using the POSE®3.0 Endo-sleeve procedure (English: “Primary Obesity Surgery Endoluminal”) is an endoscopic option for medically supporting weight loss.

POSE® 3.0 endo-sleeve: stomach reduction without surgery

In the POSE® 3.0 procedure, only 6–8 anchors are placed along the inner wall of the stomach, instead of the previous 14–16. Thanks to the double-fold strategy and double-helix technology, each anchor grips two fold layers and achieves a stronger gathering.

  • 6–8 anchors instead of 14–16, for an even gentler treatment
  • Double-fold strategy for greater tissue uptake
  • Double-helix technology holds double fold layers
  • Volume reduction: the aim is for only approx. 25% of the stomach volume to remain

Minimally invasive, as an outpatient or with one overnight stay, and with short downtime. Find out more at the Westfalenklinik Dortmund and arrange a consultation appointment.

How is the stomach reduced with the Endo-sleeve?

The stomach reduction is achieved endoscopically through the oesophagus directly in the stomach by the targeted placement of so-called anchors using special sutures. Depending on the patient’s size, starting weight and stomach volume, the doctor performing the treatment places about 6 to 8 small anchors in order to create volume-reducing folds in the stomach and thus make the stomach smaller. The POSE®3.0 Endo-sleeve procedure works at several points:

  1. Through reducing the stomach volume in the upper part of the stomach to reduce food intake and make you feel full sooner;
  2. Through narrowing the stomach outlet to prolong the feeling of fullness and delay the onset of hunger;
  3. Through shaping the bulging middle part of the stomach into a considerably slimmer, more tube-like structure.

This means your stomach can be permanently reduced by up to 75%. All that remains is an elongated, narrow stomach shape. Nothing is removed: the normal properties of the stomach and its function remain untouched. Regular and long-term intake of accompanying medication or food supplements is not necessary.

The procedure in detail

The doctor performing the treatment introduces tiny instruments through the mouth into the stomach and, during a gastroscopy, to reduce the stomach volume, first creates a number of folds in the stomach wall in the area of the stomach that stretches particularly strongly when food is eaten. Published studies show that forming folds in precisely this part of the stomach can help you feel full after just a small snack and feel full sooner when eating a daily meal.

In the new POSE® 3.0 procedure, we now place only 6–8 special anchors in a double-fold strategy. Each anchor grips two layers of stomach wall (“Double Helix”), achieving a stronger gathering than before.

Afterwards, the treating doctor repeats this process from the lower end of the stomach as well. This slows the emptying of the stomach, and the feeling of fullness after a meal lasts longer before the next hunger sets in.

Endo-sleeve, gastric balloon or surgery?

The minimally invasive POSE®3.0 Endo-sleeve procedure is a gentle procedure in obesity therapy. Studies report that excess weight is reduced by an average of 50 to 60 percent within two years; how much weight you lose yourself depends on your starting situation and on changes to your diet. Your doctors at the Westfalenklinik help with an individually tailored, healthy change in diet.

The procedure combines the advantage of the permanence of a surgical measure with the advantages of a gastroenterological examination. It is carried out without an incision through the abdominal wall and, compared with other, considerably more extensive procedures, offers a quick recovery after the procedure. The vast majority of patients are back at work and going about their normal daily routine after a long weekend. A gastric balloon, by contrast, reduces the stomach only temporarily.

Advantages of the POSE®3.0 Endo-sleeve procedure

  • A permanent stomach reduction can bring about better control of the feeling of hunger
  • Treatment is carried out without an incision through the abdominal wall
  • The POSE®3.0 Endo-sleeve allows a quick recovery and can therefore be carried out without the people around you noticing
  • The procedure avoids the wound risks of other surgical procedures for stomach reduction

Who is the Endo-sleeve an option for?

The Endo-sleeve is generally an option for patients with a BMI of 30 to 40 kg/m² who are looking for an endoscopic alternative to gastric sleeve surgery. What matters is that you are ready to change your diet and lifestyle for the long term: how much weight you lose and keep depends essentially on how consistently you follow the recommendations. Before the procedure, there is a detailed briefing and an examination at the Westfalenklinik, during which we also clarify whether there are any health reasons against the procedure.

    Beforehand

Consultation in Dortmund. In the consultation we examine you, discuss your goals and clarify whether the Endo-sleeve is an option for you. You can book your appointment online.

  2. Step 2

    Day of the procedure

The gastroscopy. The procedure takes about 45 minutes and is carried out under general anaesthesia. Afterwards you go home or stay in the clinic for one night.

  3. Step 3

    The first few days

Rest. Nausea, vomiting and weakness are possible; medication and, if needed, infusions relieve the symptoms.

    After 3 to 4 days

Back to everyday life. Most patients resume their daily routine after a long weekend.

  5. Step 5

    After the first few weeks

Normal eating, in smaller portions. With accompanying nutritional advice, you return to a diet without dietary restrictions.

How safe is the Endo-sleeve?

The POSE® and POSE®3.0 Endo-sleeve procedures have already been carried out more than 13,000 times worldwide and are still specifically accompanied by medical studies today. To date, compared with many other stomach reduction procedures, this method shows good results and, above all, greater patient safety, because it does not involve the risks of classic procedures of obesity surgery. Like any procedure, however, the Endo-sleeve can also have side effects and complications; we inform you about these in detail before the treatment.

Studies are also available on the results in terms of weight loss: in clinical studies, POSE® patients lose on average considerably more of their initial or excess weight than with other minimally invasive procedures. How much you yourself will lose cannot be predicted.

In addition, the dreaded yo-yo effect can be reduced, because the volume of the stomach is permanently reduced. While with conventional therapies cravings return quite quickly, increasing the risk of renewed weight gain, this was observed considerably less often with the POSE® procedure. Whether renewed weight gain is avoided, however, depends essentially on your diet and lifestyle.

The Westfalenklinik’s experience with the POSE method

The team at the Westfalenklinik Dortmund has been working with the POSE® procedure for years and today with the POSE® 3.0 Endo-sleeve. In a personal consultation, our doctors will be happy to give you an insight into their many years of experience with the POSE® and POSE®3.0 Endo-sleeve procedures.

What does the POSE® 3.0 Endo-sleeve cost in Dortmund? Costs and German health insurance

For self-pay patients, the treatment costs of the POSE® 3.0 Endo-sleeve start from €9,990. The price may vary depending on location and care model. A transparent treatment and cost plan provides financial clarity before the procedure. The Westfalenklinik's treatment prices are final prices including statutory VAT; overnight stay, meals, follow-up treatments and the costs of the anaesthetist are not added afterwards. Financing is possible through our financing partner; find out more under Financing your treatment.

POSE® 3.0 Endo-sleeve and German statutory health insurance: when the costs can be covered

Whether German statutory health insurance covers the costs of the POSE® 3.0 Endo-sleeve depends on clear medical guidelines. In principle, reimbursement may be possible if obesity therapy is medically necessary and other measures have not achieved lasting success.

Requirements for possible cost coverage by German statutory health insurance

The German statutory health insurance funds review applications on the basis of fixed guidelines. Cost coverage is only an option if the following criteria are met:

  • Body mass index (BMI) of 40 or more, or 35 or more with concomitant conditions such as type 2 diabetes, high blood pressure or sleep apnoea.
  • Evidence of conservative therapies carried out without success for at least six months, such as nutritional advice, exercise and behavioural therapy.
  • Submission of a medical report confirming the medical necessity of the endoscopic stomach reduction.

This ensures that the POSE® 3.0 Endo-sleeve is assessed as a medically indicated therapy for the treatment of obesity and not as an aesthetic procedure.

The multimodal concept (MMK) as the key to approval

Central to approval by German statutory health insurance is proof of structured pre-treatment as part of the multimodal concept (MMK). This programme documents that patients are actively involved in their weight reduction and receive medical support.

The MMK consists of 5 pillars:

  1. Nutritional advice (5 sessions): Professional nutritional advice from certified specialists, documentation of eating habits and preparation for eating after the procedure.
  2. Exercise log: An individually tailored exercise programme that strengthens metabolism and endurance and documents physical activity.
  3. Psychological assessment: Psychological support to rule out eating disorders and to ensure psychological stability, as well as support with behavioural changes.
  4. Gastroscopy (examination of the stomach): Checking the health of the stomach and ruling out contraindications.
  5. Full blood count: Comprehensive laboratory testing to identify deficiencies and as a basis for later aftercare.

Participation in an MMK programme is regarded as a decisive criterion for a positive decision on costs. Only when all evidence is complete and confirmed by a doctor does the likelihood increase that German statutory health insurance will support the treatment. You can read further details under Costs and German health insurance.

Private health insurance and self-pay patients

Private insurers decide on cost coverage on a case-by-case basis. If the POSE® 3.0 Endo-sleeve is classified as medically necessary, many companies cover the costs in part or in full. Without cost coverage, the Endo-sleeve is a self-pay service at the prices stated above.

Frequently asked questions

What advantages does the POSE®3.0 Endo-sleeve procedure offer me?

Stomach reduction using the POSE®3 Endo-sleeve procedure is designed to be permanent and is thus intended to help the patient with weight loss and changing their lifestyle habits over a considerably longer period. While a gastric balloon can be worn for a maximum of 12 months and other surgical operations entail considerably greater risks and side effects, the POSE®3 procedure can be a compromise for severely overweight and obese people. Which procedure is an option for you is something we discuss in the consultation.

Who is an option for treatment using the POSE®3.0 Endo-sleeve procedure?

Patients with a BMI of 30–40 kg/m² who are looking for an alternative to gastric sleeve surgery without an incision through the abdominal wall and are ready to change their diet and lifestyle for the long term.

What weight loss is achievable with the POSE®3.0 Endo-sleeve procedure?

A loss of up to 25% of the initial weight is cited, which corresponds to about 60% of the excess weight. However, this always depends on the individual condition of the patients and their willingness to follow the recommendations of our doctors and to pursue a new lifestyle in the long term. With the POSE®3 procedure, our patients now have considerably more time to change their habits and to reduce their weight in an even more controlled way. This can counteract the dreaded yo-yo effect.

How long do I need after a stomach reduction using the POSE®3.0 Endo-sleeve procedure until I am fully fit again?

One advantage of the POSE®3 Endo-sleeve procedure is that most patients are fully fit again and able to manage their daily routine after a long weekend, i.e. after 3–4 days. With other comparable surgical measures aimed at stomach reduction, you sometimes need months to be fit again and have to live with considerable restrictions.

What is the “Double Helix” effect?

With POSE 3.0, each anchor holds two overlapping folds at the same time. This allows more tissue to be captured and the stomach to be gathered more effectively, with fewer sutures and greater stability.

What is an Endo-sleeve?

An Endo-sleeve is an endoscopic stomach reduction. During a gastroscopy, the stomach is gathered from the inside with anchors and sutures, becoming narrower and taking on a tube-like shape. Nothing is removed in the process and the abdominal wall is not opened. At the Westfalenklinik, we work with the POSE® 3.0 technique.

How long does the procedure take?

About 45 minutes. It takes place under general anaesthesia as part of a gastroscopy. Afterwards you can go home or stay in the clinic for one night.

What side effects and risks does the Endo-sleeve have?

The Endo-sleeve is also a medical procedure. In the first few days, severe nausea and vomiting, dehydration and weakness may occur; you receive medication for this and, if needed, infusions. Like any procedure, it can also have complications. We inform you about all risks in detail before the procedure.

Does German statutory health insurance cover the costs?

Reimbursement may be possible if obesity therapy is medically necessary and other measures have not had lasting success, for example with a BMI of 40 or more, or 35 or more with concomitant conditions. Proof of a multimodal concept is central. Private insurers decide on a case-by-case basis.

What does an Endo-sleeve cost in Dortmund?

For self-pay patients, the treatment costs start from €9,990. After your consultation at the Westfalenklinik, you receive a treatment and cost plan. The prices are final prices including VAT; financing is possible through our financing partner.
