Lipoedema surgery: when does German statutory health insurance cover the costs?

Lipoedema surgery is a big step for many of those affected – but who bears the costs? Whether you have German statutory or private health insurance: under certain conditions, the costs may be covered. Here you can find out what you need to know.

The key points in brief

  • Private health insurance (PKV)Check your tariff for exclusions of benefits. With a specialist’s diagnosis, documented conservative therapy and weight stability, the chances of cost coverage are good.

  • German statutory health insurance (GKV)Since 2024, liposuction has been covered by statutory health insurance under strict requirements – the decisive factors are diagnostic criteria, BMI limits and 6 months of prior treatment.

  • Self-pay patientsAt Westfalenklinik Dortmund you receive an individual treatment quote – regardless of health insurance requirements. Financing is possible through our financing partner.

Cost coverage by private health insurance (PKV)

As a private patient, cost coverage depends on your individual insurance tariff. Before you submit an application, you should clarify: is lipoedema included in your schedule of benefits? Are there exclusions for certain treatments? If in doubt, it is worth speaking directly to your insurer.

These are the documents you need for the application

  1. Step 1

Specialist diagnosis by doctors who do not operate

The initial diagnosis must be made by a specialist in internal medicine and angiology, physical and rehabilitative medicine, dermatology or a doctor with additional training in phlebology.

  2. Step 2

    Documentation of conservative therapy

At least six months of medically supervised treatment (e.g. compression, lymphatic drainage) must be documented without gaps.

  3. Step 3

    Proof of weight stability

Monthly weight checks at your GP document that your weight has remained stable over the treatment period.

  4. Step 4

Medical certificate on the indication for surgery

A specialist in plastic, reconstructive and aesthetic surgery confirms the medical necessity of the liposuction.

  5. Step 5

    Submission to the insurer

The cost estimate and all evidence of therapy are submitted together to your private health insurer (PKV). By the way: some insurers, such as Hanse Merkur, offer special programmes for patients with lipoedema – ask about them specifically.

Cost coverage by German statutory health insurance (GKV)

German statutory health insurance (GKV) covers the costs of lipoedema surgery under clearly defined requirements. The path to approval is more strictly regulated than with PKV – but cost coverage is possible in principle.

Diagnostic requirements

For recognition by the GKV, the following findings must be present:

  • Symmetrical increase in fatty tissue on the arms and/or legs
  • Hands and feet remain slim and are not affected
  • Pain on pressure and touch in the affected tissue

Medical requirements for liposuction

All of the following criteria must be met:

  • Conservative therapy for at least 6 months without sufficient improvement
  • No weight gain in the last 6 months (monthly documentation)
  • Psychological factors have been recorded by a doctor and taken into account

Financing surgery for self-pay patients

As a self-pay patient, you do not have to go without your lipoedema treatment. Financing is possible through our financing partner: you can pay for your surgery in instalments. This makes the treatment financially plannable even without health insurance coverage.

BMI limits for GKV cost coverage

BMI 32–35 kg/m²

Liposuction is only possible if the excess weight is predominantly caused by the lipoedema (observe WHtR limits).

BMI of 35 kg/m² or more

Treatment for obesity is required first before liposuction can be approved.

BMI of 40 kg/m² or more

No liposuction possible via the GKV – as a self-pay patient, we advise you on alternatives.

Who is allowed to perform lipoedema surgery?

Liposuction may only be performed by qualified specialists: specialists in plastic, reconstructive and aesthetic surgery, surgical specialists from other surgical disciplines and dermatologists. From January 2026, the Kassenärztliche Vereinigung will publish an official list of approved practitioners.

What does health insurance cover – and what not?

Funded by German statutory health insurance

  • Liposuction of the arms, legs and, where applicable, buttocks
  • Anaesthesia and inpatient monitoring

Frequently asked questions

How much does lipoedema treatment cost?

The costs of lipoedema surgery depend on the extent of the treatment, the number of areas of the body affected and the stage of the lipoedema. The more pronounced the findings and the more regions need to be treated, the more complex the procedure. At Westfalenklinik Dortmund you receive an individual treatment quote as part of your first consultation. Financing is possible through our financing partner – so the treatment can be planned even without cover from German statutory health insurance.

Which doctor is good for lipoedema?

The diagnosis is the responsibility of specialists in internal medicine and angiology, physical and rehabilitative medicine, dermatology or doctors with additional training in phlebology. The surgical treatment should be performed by an experienced specialist in plastic and aesthetic surgery who specialises in liposuction. At Westfalenklinik Dortmund, doctors such as Dr. med. Christian Josephs have been treating lipoedema patients for years – with personal care from the first consultation to aftercare.

Which German statutory health insurer pays for lipoedema surgery?

Since 2024, liposuction for lipoedema has been a benefit of German statutory health insurance under certain conditions. All statutory health insurers are obliged to cover the costs if the criteria are met: a diagnosis by a specialist doctor, 6 months of conservative therapy without sufficient improvement, weight stability and compliance with the BMI limits. With private health insurance, coverage depends on the individual policy – some insurers such as Hanse Merkur even offer special lipoedema programmes.

Does my German statutory health insurance cover lipoedema surgery?

With German statutory health insurance, cost coverage is possible under certain conditions: a diagnosis by a specialist doctor, documented conservative therapy over 6 months, weight stability and compliance with the BMI limits. With private health insurance, it depends on your individual policy.

Which doctors may make the lipoedema diagnosis for German statutory health insurance?

Only specialists in internal medicine and angiology, physical and rehabilitative medicine, dermatology or doctors with additional training in phlebology. GPs and plastic surgeons are not authorised to make the initial diagnosis.

What do I need to document for the conservative therapy?

The conservative pre-treatment must be prescribed by a doctor and carried out continuously for at least 6 months. The documentation includes: regular compression garments, manual lymphatic drainage, exercise therapy and, where applicable, dietary adjustment. It should be possible to provide complete proof of all measures – keep prescriptions, treatment reports and therapy plans carefully.

What does weight stability mean and how do I prove it?

Weight stability means that you have not gained weight in the last 6 months before the application. Proof is provided through monthly weight checks at your GP, which must be documented in writing. This documentation is an important part of the application for both German statutory health insurance and private health insurance.

Are all stages of lipoedema covered by German statutory health insurance?

In principle yes, but with restrictions: for lipoedema at stage I and II, treatment is currently provided on an outpatient basis only, with a maximum suction volume of three litres. For advanced stages, more extensive procedures may be necessary – here an individual review by the insurer is required.

Is skin tightening after liposuction covered?

No, skin tightening is funded neither by German statutory health insurance nor by private health insurance. As a self-pay patient, however, you can have this treatment carried out at Westfalenklinik – for example a thigh lift or upper arm lift.

Can I still be treated with a higher BMI?

From a BMI of 35 kg/m², obesity treatment is required first before liposuction through German statutory health insurance is possible. As a self-pay patient, we will be glad to advise you on your individual options.
