Motiva Preservé™ or classic breast surgery: the methods compared

The decision to have a breast augmentation is far more than a question of cup size – it is the choice between two fundamental surgical philosophies. While proven standards offer a great deal of shaping flexibility, the minimally invasive technique focuses on preserving tissue and a shorter recovery time.

The key points in brief

  • Minimal invasivenessThe Motiva Preservé™ technique shortens the duration of surgery to approx. 30 minutes and uses micro-incisions of 2–2.5 cm to stretch the tissue in a controlled way instead of cutting through it extensively as in the classic procedure.

  • Fast recoveryThanks to the no-touch technique and the option of local anaesthesia, patients are often able to manage everyday life again after just 1–2 days, whereas classic methods usually require a week of recovery and one night as an inpatient.

  • Tissue preservationBy consistently preserving natural supporting structures and nerves, the Preservé™ method aims to reduce the risk of capsular contracture or implant displacement.

Deciding on a breast augmentation is far more than a question of cup size – it is a choice between two fundamental surgical philosophies. While established standards offer great scope for shaping, the minimally invasive technique focuses on preserving tissue and a shorter recovery time. Will you opt for the surgical precision of the classic approach or the quick recovery of the modern one? Here you can find out which path suits your individual safety requirements and aesthetic goals in 2026.

Motiva Preservé™ and classic breast surgery: the methods compared

Breast augmentations are among the most searched-for aesthetic procedures in Germany. The term “breast augmentation” alone is searched for around 26,810 times per month on average. That is exactly why the market is so clearly divided: many women want a beautiful result – at the same time, the desire for as gentle a procedure as possible with short downtime is growing. This is where Motiva Preservé™ comes in: a minimally invasive, tissue-preserving technique that deliberately sets itself apart from classic breast surgery.

The most important point first: this is not about “good” versus “bad”. It is about two surgical philosophies that prioritise different goals. One relies on great variability and established standards (classic breast surgery). The other relies on tissue preservation, small access incisions and quick recovery (Motiva Preservé™). Which method suits you depends less on trends – and more on anatomy, your goals and safety requirements.

What is Motiva Preservé™?

Motiva Preservé™ is a new, minimally invasive technique for breast augmentation. The core: the implant is inserted through a small incision of about 2–2.5 cm in the inframammary fold. Instead of cutting or detaching tissue extensively, it is stretched in a controlled way. As a result, supporting structures and nerves more often remain intact – an important factor if you value sensation and smooth healing.

Another component is the no-touch technique: the implant is inserted in a way that reduces direct contact with the skin. The aim behind this is clear: to reduce the risk of infection, make placement more precise and irritate the tissue as little as possible.

Another typical feature of Preservé™ is the use of Ergonomix2® implants. These implants are designed to adapt to body posture and are therefore often perceived as particularly natural – both visually and in the way they “move with” the body.

Motiva Preservé™ “in a nutshell” (brief, for orientation)

  • Incision

approx. 2–2.5 cm in the inframammary fold

  • Duration of surgery

    approx. 30–45 minutes

  • Anaesthesia

    Local anaesthesia / twilight sedation

  • Everyday life

often easily possible again after 1–2 days

  • Aim

tissue preservation, small scars, natural look, lower risk of capsular contracture/implant displacement

What is classic breast surgery?

In classic breast augmentation, the implant pocket is usually prepared by cutting and detaching tissue. This is the big difference: the pocket is usually created through more extensive dissection, which gives the surgeon a lot of control – but also puts more strain on the tissue.

The operation typically takes 60–90 minutes, is performed under general anaesthesia and often requires one night as an inpatient. Because the procedure is more invasive, the recovery phase is usually longer: 5–7 days until you are fit for everyday social life is realistic, and sport tends to make sense again only after 4–6 weeks.

The incision is also longer by comparison: often 4–6 cm. This increases the likelihood that scars will be more noticeable – depending in particular on skin type, healing and aftercare.

  • Incision

    often 4–6 cm

  • Duration of surgery

    approx. 60–90 minutes

  • Anaesthesia

    General anaesthesia

  • Clinic

    often inpatient, 1 night

  • Everyday life/sport

everyday life often after 5–7 days, sport usually after 4–6 weeks

  • Risks

tend to be higher (e.g. capsular contracture, infection, rupture, displacement), pain often more severe

Which method makes sense for whom?

This is where theory parts ways with real surgical planning. Not every anatomy automatically “allows” the minimally invasive route – just as classic surgery is not automatically “too much”. What matters are the goal, the tissue, the breast base and the initial shape.

Motiva Preservé™ can be a particularly good fit if …

you would like a gentle breast augmentation that slows your body down as little as possible. This is often the case if you would like more volume again after pregnancy or weight loss, but cannot or do not want to plan for long downtime. If you want to avoid general anaesthesia, Preservé™ is also interesting in principle, because the procedure is usually possible with local anaesthesia and twilight sedation.

What often speaks in favour of the method in practice is the combination of a small scar, faster recovery and the feeling that the result moves “with the body” – not like a rigid element.

Classic breast surgery often has the advantage if …

you would like more volume, have a more complex breast shape or need a correction that requires considerably more surgical shaping. As soon as pocket creation, implant position or asymmetry correction have to be controlled very precisely across several planes, classic methods often offer the bigger toolkit. This can also apply if the starting point is very slim or soft-tissue coverage is thin and the implant position has to be planned more strategically.

In short: Preservé™ is strong when “gentle and quick” is realistic. Classic is strong when “shapeable and versatile” is needed.

Risks: what really differs – and what does not

Whichever method you choose, a breast augmentation remains a surgical procedure. Basic risks such as swelling, bruising, wound healing problems or infections are possible in principle.

The difference lies more in the likelihood and course of certain issues:

  • With Preservé™, the tissue is dissected over a smaller area. This can mean that swelling and discomfort are less pronounced and healing seems “calmer”.
  • The combination of tissue-sparing pocket creation and the no-touch principle aims to reduce the risk of infection and capsular contracture.
  • In conventional operations, the dissection is more extensive. As a result, pain can be more pronounced, and certain risks (e.g. capsular contracture or implant displacement) tend to be an issue more often – not because the method is “worse”, but because more tissue is worked on and the operation is more invasive overall.

Healing: what you should realistically plan for

Many decisions hinge on one point: everyday life. Anyone who looks after children, does physical work or needs to be back on their feet quickly will assess the recovery time differently from someone who can easily arrange a break of several weeks.

With Motiva Preservé™, recovery is often faster: many patients feel able to manage everyday life again after 1–2 days. Sport is typically possible again from week 2–3 – sensibly, of course, with moderation and depending on how things progress.

With conventional breast surgery, the downtime is usually longer: 5–7 days is often the phase after which you “really function” again without feeling every movement. Sport is more of a topic for week 4–6, depending on the implant position and strain.

What applies in both cases: the final “settling” and the final shape develop over weeks. At first, the breasts can look higher, feel firmer and only settle step by step.

Costs: why Preservé™ is often higher – and why that is not everything

As a rough guide, the Westfalenklinik quotes a starting price from €9,800. This does not replace an individual calculation. In practice, the final price depends above all on these points:

  • Implant type and size
  • Type of anaesthesia (local anaesthesia/twilight sedation vs. general anaesthesia)
  • Duration of surgery and effort of pocket creation
  • Stay in the clinic (outpatient vs. inpatient)
  • Aftercare concept (check-ups, compression bra, medication)

A fair comparison looks not only at the price, but also at what you “buy along with it”: downtime, comfort, intensity of aftercare and safety concept. Financing is possible through our financing partner.

Conclusion: which method suits you?

Motiva Preservé™ stands for a modern, tissue-sparing breast augmentation with a small incision (2–2.5 cm), short operating time (approx. 30 minutes) and often a quick return to everyday life (1–2 days). The method aims to preserve sensitivity and supporting structures as far as possible and to reduce risks such as capsular contracture or implant displacement.

Conventional breast surgery remains the right option when the starting situation calls for more surgical shaping: wishes for larger volumes, more complex shapes, corrections or situations in which more extensive pocket dissection is simply necessary. In return, you must expect general anaesthesia, a longer duration of surgery (60–90 minutes) and a longer recovery phase.

If you want clarity, what counts in the end is not an internet comparison but the question: What is realistic for your anatomy – and which method will take you to your goal with the lowest risk? That is exactly what Dr. med. univ. Nura Zamani of the Westfalenklinik clarifies with you in a personal consultation – with a view to shape, safety and a healing process that fits your everyday life.

Frequently asked questions

Is the Motiva Preservé™ method painful?

Many patients find Preservé™ less stressful because the tissue is not cut through over a large area but stretched in a controlled way. That does not mean you feel nothing – but the start is often “easier” than with a conventional operation, especially if no major involvement of the muscle is needed.

How long does the result last?

Implants are not a “once-and-for-ever” decision, but neither do they automatically mean an early exchange. What matters are check-ups, how the tissue behaves, the implant type and individual factors. The consultation typically also covers how you can check in the long term whether everything remains stable.

Can breastfeeding be possible afterwards?

In principle, this depends on the incision, implant position and initial anatomy. With tissue-sparing approaches, the preservation of structures is more often discussed as an advantage. A concrete assurance can only be given responsibly after examination and planning.

How visible are scars really?

With Preservé™, the incision is very small and lies in the inframammary fold, which often leads to inconspicuous scars. With conventional operations, the incision is longer, which means scars can tend to be more visible – but skin type, aftercare and individual tendency to scarring also play a decisive role.
