Lipoedema surgery with German statutory health insurance in Dortmund: consultation and aftercare at Westfalenklinik

With German statutory health insurance, your path is divided:

  • Consultation and examination: Westfalenklinik, Dortmund
  • Preparation and application: Westfalenklinik, Dortmund
  • Operation: coordinated through our Nationalklinik in Wertheim
  • Aftercare and check-ups: Westfalenklinik, Dortmund

As a self-pay patient, by contrast, you have your surgery at the Westfalenklinik itself, more on this under Lipoedema surgery as a self-pay patient.

Many women with lipoedema have worn compression and attended lymphatic drainage for years before the question of an operation arises. Since German statutory health insurance can cover liposuction in principle at any stage, a procedure is within reach for considerably more of those affected. Today, the hurdles lie above all in the requirements that must be met and documented.

At Westfalenklinik you receive a well-founded assessment of your situation, medically and organisationally. In the consultation we examine you, check together with you which requirements are already met and what is still missing, and plan the next steps. We check your entitlement and guide you through the application to your German statutory health insurer.

You will find us at Rosemeyerstraße 2–4, directly on the B1/A40 in the immediate vicinity of the Signal Iduna Park. By car, leave the B1 at the Dortmund-Hombruch exit and park on the building’s car park deck directly in front of the entrance. From the main station, the U45 takes you to Stadtgarten, and from there you take the U42 to Theodor-Fliedner-Heim. This way the appointments before and after the procedure also remain easy to reach.

Since October 2025: liposuction for lipoedema covered by health insurance

On 17 July 2025, the Gemeinsamer Bundesausschuss (G-BA, Federal Joint Committee) decided to include liposuction for lipoedema in the standard benefits catalogue of German statutory health insurance. The decision has been in force since 9 October 2025. This removed the former restriction to stage 3: since then, liposuction can in principle be a statutory health insurance benefit at any stage. Since 1 July 2026, doctors contracted with statutory health insurance have also been able to bill for it accordingly in stages 1 and 2.

The decisions of the G-BA apply to all statutory health insurance funds. Whether you are insured with Techniker Krankenkasse, an AOK, Barmer, DAK or a company health insurance fund makes no difference to the rules: all statutory health insurance funds are obliged to cover the costs if the criteria are met. Whether they are met in the individual case is checked by the health insurance fund.

The health insurance requirements at a glance

For health insurance to be able to cover lipoedema surgery, all of these points must apply:

  • Confirmed diagnosis by a non-operating specialist doctor authorised for this
  • At least 6 months of conservative therapy prescribed by a doctor without sufficient improvement
  • No weight gain in these 6 months, documented monthly
  • BMI under 35, between 32 and 35 with an age-dependent limit on WHtR
  • Psychological factors have been recorded by a doctor and taken into account
  • Diagnosis and the decision for surgery by different doctors

For recognition, typical findings must be present: a symmetrical increase in fatty tissue on the arms and/or legs, slim, unaffected hands and feet, and pain on pressure and touch in the affected tissue. The diagnosis for the insurer is made by specialists in internal medicine and angiology, in dermatology, in physical and rehabilitative medicine, or by doctors with additional training in phlebology; family doctors and plastic surgeons are not authorised to make the initial diagnosis. How the examination works, you can read under Lipoedema diagnosis in Dortmund. What belongs to conservative therapy is explained under Treating lipoedema without surgery.

Surgery may be performed by specialists in plastic, reconstructive and aesthetic surgery, specialists in other surgical disciplines and dermatologists. From January 2026, the Kassenärztliche Vereinigung publishes an official list of authorised practitioners.

BMI and WHtR: What counts for your lipoedema surgery under health insurance

BMI alone does not decide, but it sets the framework: if it is between 32 and 35, liposuction is only possible if the excess weight is mainly due to the lipoedema. Waist circumference in relation to height then also counts.

You can calculate your BMI and your WHtR with our WHtR calculator. The calculator also shows which limit applies for your age. The binding assessment is made in the medical examination.

BMI and WHtR

What BMI means for the health insurance benefit

Three BMI ranges determine whether it stands in the way of lipoedema surgery under health insurance.

Scale in three sections, the pointer is in the first section

BMI under 32no obstacle

BMI does not stand in the way of health insurance coverage. The other requirements count: diagnosis, conservative therapy and a stable weight.

BMI 32 to 35WHtR also counts

Liposuction is only possible as a statutory health insurance benefit if the excess weight is mainly due to the lipoedema. The waist-to-height ratio (WHtR) also counts for this: it must not exceed an age-dependent limit. Our WHtR calculator shows which limit applies to your age.

BMI of 35 or moreweight first

Treatment of the excess weight is required first before liposuction can be approved; we also advise you on this under weight loss. From a BMI of 40, liposuction via health insurance is not possible; as a self-pay patient, we advise you on alternatives.

Stage 1, 2 or 3: What health insurance covers today

Since October 2025, the stage no longer decides whether health insurance pays. That fits the current guideline: the stages describe how the skin and fatty tissue look, not how severe the symptoms are. Whether liposuction makes sense depends on your symptoms and on how they respond to conservative treatment. For lipoedema in stages 1 and 2, treatment via health insurance is currently only carried out on an outpatient basis, with a suction volume of no more than three litres. What the individual stages describe, you can read under Lipoedema symptoms and stages.

Your path to lipoedema surgery with German statutory health insurance in Dortmund

  1. Step 1

    First assessment

Consultation at Westfalenklinik. We examine you, talk about your symptoms and previous treatments, and show you which requirements are already met.

  2. Step 2

    Confirmed diagnosis

Specialist findings. A specialist doctor authorised for this confirms the diagnosis for the insurer. We tell you who qualifies for this.

  3. Step 3

    At least 6 months

Conservative therapy. Compression, lymphatic drainage and exercise, prescribed by a doctor and documented. Your weight should not increase during this time; your family doctor’s practice records it monthly.

  4. Step 4

    Second medical review

Decision on surgery. If the symptoms persist, a different doctor from the one who made the diagnosis checks whether liposuction is indicated.

  5. Step 5

    Application and procedure

Surgery via the Nationalklinik in Wertheim. We support you through the application to your health insurance fund and plan together which body regions will be treated and whether several sessions make sense. We coordinate the procedure via our Nationalklinik in Wertheim.

  6. Step 6

    Afterwards

Aftercare in Dortmund. For check-ups and aftercare, you come back to Westfalenklinik.

  • Prescriptions for compression and lymphatic drainage
  • Proof of at least 6 months of conservative therapy
  • Your weight record over this period, documented monthly
  • Height, weight and waist circumference for BMI and WHtR
  • Earlier doctors’ letters and findings

Bring what you already have to your consultation in Dortmund. We will discuss with you what is still missing.

What German statutory health insurance covers and what it does not

Statutory health insurance finances liposuction on the arms, legs and, where applicable, the buttocks, the anaesthesia and inpatient monitoring, as well as standard aftercare.

Not covered are liposuction on the abdomen, back or chest, skin tightening after liposuction and, as things currently stand, treatment in private clinics. A tightening procedure, for example on the thighs or upper arms, therefore remains a self-pay service; more on this under Skin tightening after lipoedema surgery.

Privately insured?

As a private patient, cost coverage depends on your individual insurance plan. Before applying, clarify whether lipoedema is included in your benefits catalogue and whether there are exclusions for certain treatments; if in doubt, it is worth speaking directly to your insurer. You submit the cost estimate and all proof of therapy together to your insurer. Some insurers, such as Hanse Merkur, offer special programmes for lipoedema patients; ask specifically about them.

What does lipoedema surgery cost in Dortmund if German statutory health insurance does not pay?

Not every patient meets all the requirements or wants to wait out the period of prior treatment; what applies then is explained under Lipoedema surgery as a self-pay patient. As a self-pay patient, you are operated on at Westfalenklinik itself, without the requirements of health insurance, but not without an examination: whether lipoedema surgery is an option for you, we decide together.

The costs depend on the extent of the treatment, the number of affected body regions and the stage. After the consultation you receive a no-obligation treatment quote; the prices are final prices including statutory VAT, and accommodation, meals, follow-up treatments and anaesthesia are not added afterwards. Financing is possible through our financing partner. You can read more on costs and health insurance in the article Lipoedema surgery: when does German statutory health insurance cover the costs?.

Frequently asked questions

Does German statutory health insurance pay for lipoedema surgery?

Yes, that is possible. With the decision of the Gemeinsamer Bundesausschuss (Federal Joint Committee) of 17 July 2025, in force since 9 October 2025, statutory health insurance can in principle cover liposuction for lipoedema at any stage if the requirements are met. If cost coverage is an option, we coordinate the procedure via our Nationalklinik in Wertheim; you attend consultation, preparation and aftercare at Westfalenklinik in Dortmund.

Do the rules apply equally to TK, AOK, Barmer and DAK?

Yes. The decisions of the Gemeinsamer Bundesausschuss (Federal Joint Committee) apply to all statutory health insurance funds, whether Techniker Krankenkasse, AOK, Barmer, DAK or a company or guild health insurance fund. All statutory health insurance funds are obliged to cover the costs if the criteria are met. What matters is your medical requirements, not the name of your insurer.

Do I have to submit an application to my health insurance?

We check your entitlement with you and support you through the application to your health insurance fund. Your documents on diagnosis, conservative therapy and weight are important for this. So keep prescriptions, treatment reports and therapy plans carefully.

Which BMI applies to lipoedema surgery under health insurance?

Your BMI must be under 35. Between 32 and 35, liposuction is only possible if the excess weight is mainly due to the lipoedema; the waist-to-height ratio (WHtR) must then not exceed an age-dependent limit. From a BMI of 35, treatment of the excess weight is required first; from 40, liposuction via health insurance is not possible. Our WHtR calculator works out your value.

Does health insurance also pay for lipoedema stage 1 or 2?

Yes. Until October 2025, cost coverage was limited to stage 3; this restriction no longer applies. Since 1 July 2026, doctors contracted with statutory health insurance have also been able to bill for liposuction accordingly in stages 1 and 2. Whether an operation is an option depends on your symptoms and the other requirements.

What counts as conservative therapy?

This mainly includes regular compression, manual lymphatic drainage, exercise therapy and, where necessary, an adjustment of your diet. The therapy must be prescribed by a doctor, last continuously for at least 6 months and be documented without gaps. More on this under Treating lipoedema without surgery.

How do I prove that my weight is stable?

Weight stability means that you have not gained weight in the last 6 months before the application. It is proven by monthly weight checks with your family doctor, which are documented in writing.

Does health insurance also cover skin tightening after lipoedema surgery?

No. Skin tightening is not financed by either statutory or private health insurance. As a self-pay patient, you can have a tightening procedure on the thighs or upper arms carried out at Westfalenklinik. More on this under Skin tightening after lipoedema surgery.

What applies to privately insured people?

With private health insurance, cost coverage depends on your individual plan. Check your plan for exclusions of benefits and ask before the treatment which documents are required. Some insurers, such as Hanse Merkur, offer special programmes for lipoedema patients; ask specifically about them.

What does lipoedema surgery cost in Dortmund without German health insurance coverage?

Without health insurance coverage, you pay for the treatment yourself. The price depends on which body regions are treated and how much tissue is removed by liposuction. After the first consultation at Westfalenklinik you receive a no-obligation treatment quote at final prices; you can finance it through our financing partner.

Where does lipoedema surgery under health insurance take place?

If cost coverage is an option, we coordinate the procedure via our Nationalklinik in Wertheim. As things currently stand, statutory health insurance does not cover treatment in private clinics. Consultation, preparation and aftercare remain with us at Westfalenklinik. As a self-pay patient, you are operated on at Westfalenklinik itself.

Where in Dortmund can I get advice as a patient with German statutory health insurance?
